What is it?
We deliver personalized behavioral health support, working closely with patients and providers to improve mental well-being through compassionate, integrated care.
What services are provided?
As a patient of Integral Health, you will receive personalized and targeted treatment from a team of healthcare providers to address your specific needs. Your care team will use tools such as symptom rating scales and patient-chosen goals to regularly assess your progress and ensure that your treatment is effective.
What can I expect?
Enrollment & Assessment:  A licensed social worker (your care manager) will assess your needs to personalize services for you.
Care Plan Creation: You and your care manager will create a care plan together, which will include specific goals and strategies to achieve those goals.
Ongoing Support: Your care manager will regularly meet with you to provide support, including short-term therapy, medication linkage, improving your overall well-being, coordinating care with other healthcare providers, connecting you with community resources, providing education and training, and offering caregiver support.
Program Completion: You will complete the program after you have achieved all your personal goals.

Primary Care Physician (PCP): Oversees all patient care and ensures overall well-being.
Care Manager: Collaborates closely with the patient and PCP to implement the care plan, monitors progress, and offers counseling.
Psychiatric Consultant: Provides expert guidance by providing medication recommendations to your main doctor without direct patient contact.Operations Team: Provides administrative support to the care team.

Ghazia Tariq, LMSW
Ghazia is a skilled Licensed Master Social Worker specializing in depression, anxiety, and life transitions. She utilizes cognitive behavioral therapy, motivational interviewing, and solution-focused therapy in various settings, including hospitals and schools. Ghazia’s empathetic approach makes her relatable to clients seeking support. Outside of work, she’s an adventurous traveler and hiker, always ready for the next exploration.
Psychiatric Consultant
Richard Callado, PMHNP
Richard is a board-certified Psychiatric Nurse Practitioner with over 20 years of experience in mental health, currently specializing in interventional psychiatry and treatment-resistant conditions at Curated Mental Health in NYC. His comprehensive experience spans pediatric and adult psychiatry across multiple settings with expertise in treating complex psychiatric conditions, including treatment-resistant depression, schizophrenia, rapid cycling bipolar disorder, and anxiety disorders. He is an Associate Clinical Professor at Pace University and an Adjunct Professor at Stony Brook University. He is a lifelong martial artist with black belts in numerous styles, a former MMA and Muay Thai competitor, and enjoys traveling and trying new foods.

What is CoCm

The Collaborative Care Model (CoCM), developed by the University of Washington, integrates behavioral health into primary care using a team-based approach. It involves a primary care provider, behavioral health care managers, and psychiatric consultants working together to deliver evidence-based treatments. This model is supported by CMS billing codes and widely covered by commercial insurers, making behavioral health more accessible in the primary care setting.
